Medical services are provided and billed by our clinic and we participate in many of the major health insurance plans in the Central Oregon. We currently accept the following health plans.  Please be aware that with any health plans, there may be deductibles and copayments for which you could be responsible.

Self Pay

If you do not have insurance or if we do not take your insurance, you can still schedule a visit.

New Patient Office Visit - $275

Established Patient Office Visit - $175

Wellness Visit - $275

Submitting Claims to Insurance

If your plan is not listed, we will be unable to file a claim on your behalf. However, we can provide you with a form that you can submit to your insurance company to instruct them to send reimbursements directly to you.

Medicare

We are currently not accepting Medicare or Medicare Advantage, but this may change in the future.
